OUTREACH GERIATRIC REGISTERED NURSE TFT

Geriatric Outreach Nurse - One (1) Temporary Full Time
Approximately 6-12 months or until the previous incumbent returns

HOURS: Days, 8hr shifts
(subject to change in accordance with operational requirements)
" Currently registered and in good standing with the College of Nurses of Ontario
" Baccalaureate degree in Nursing is required
" Masters Degree or RN (EC) required
" Minimum 1-2 years Mental Health experience prefered
" Minimum 5 years of Emergency or Geriatric experience required
" Gerontology Nursing Certification preferred
" Experience in gerontological nursing
" Valid CPR and BCLS certificates are required
" Excellent skills in the following: negotiation, facilitation, communication, problem solving, interviewing,
counseling, teaching, leadership and documentation
" Knowledge of general gerontological nursing theory and practice to include knowledge specific to the
elderly and the aging process
" Knowledge of relevant drugs, side effects, medical conditions and emergency procedure
" Knowledge of geriatric psychiatry, community resources, crisis work and Regulated Health Profession
Act, Mental Health Act, Health Care Consent Act, Substitute Decisions Act, Public Hospitals Act and Long
Term Care Homes Act
" Sensitivity to multi-cultural issues, particularly with the elderly
" Understanding of standard assessment tools
" Fluent in English (Reading, writing, speaking, comprehension)
" Demonstrated ability to function as a case manager
" Able to work independently with minimal support
" Demonstrated commitment to continuing education
" Excellent attendance, punctuality and work record
" Will be required to travel between all hospital campuses and the LHIN area, valid drivers license and access to
car is also required
This position, in collaboration with the inter-disciplinary Geriatric Outreach Team, provides community outreach and follow up care to community-dwelling seniors (including those in LTC homes and various other supportive housing situations) who are 65yrs of age and older. The aim of this role, and overall service, is to maximize client functioning and promote community tenure.
This role will involve all necessary components of managing a client case load, responsibilities consistent with professional training, participating in program planning, quality improvement and development and additional duties as required.
