POSITION:
Home Dialysis Coordinator - One (1) Full-time

DEPARTMENT/CAMPUS:
Dialysis - Brampton Civic Hospital

POSTING NUMBER: 140410

HOURS: Currently, Monday - Friday, 8:00a.m-4:00pm (with flexibility) on-call after hours as necessary.
(Subject to change in accordance to operational requirements)

Job Summary:
Reporting to the Manager of the Renal Program, and working in conjunction with other members of the Dialysis team. The Home Dialysis (HD) Coordinator is responsible for addressing the needs of a unique patient population. The Home Dialysis Coordinator will be supporting the Home program at the William Osler Health Centre, including both the Peritoneal and Home Hemodialysis programs. Responsibilities include coordinating clinical activities, acts as a clinical resource/role model for other staff members, and works directly with patients and members of the team to ensure timely and appropriate treatment.

Responsibilities include but are not limited to:
o Program development
o Developing, implementing and maintaining policies/procedures
o acting as a clinical resource/role model for all staff
o participating in data management
o pursuing ongoing professional development
o all aspects of patient care

Qualifications include but are not limited to:
o Current registration with the College of Nurses of Ontario, BCLS required
o Baccalaureate degree required
o CNA Certified in Nephrology or equivalent
o 5 years previous clinical experience in Nephrology nursing, with emphasis in peritoneal
dialysis / hemodialysis
o previous experience in predialysis transplantation and hemodialysis preferred
o demonstrated computer skills using Microsoft office (Word, Excel, Access)
o demonstrated ability in adult teaching and knowledge of learning principles
o demonstrated leadership skills in program planning and development, staff development
o demonstrates excellent interpersonal communication skills with the ability to work with
a multidisciplinary team including other health team members, patient/family.
o demonstrates excellent patient assessment skills
o Ability to be flexible and effectively work in busy environment with changing priorities and deadlines
o demonstrated initiative in improving work skills and processes.
o Excellent attendance record.
