POSITION: ONA
Nurse Practitioner - One (1) Part Time A

DEPARTMENT/CAMPUS:
Aging at Home / Long Term Care Outreach - Brampton Civic Hospital

HOURS:
Currently, Weekend Days, flexibility in hours and shifts will be required
(Subject to change in accordance with operational requirements)

QUALIFICATIONS:
" Masters Degree in Nursing / Current extended class registration RN (EC) required with CNO
" Demonstrated patient/family teaching and staff education/ exemplary clinical skills, knowledge and expertise
" Recent and Relevant experience practicing as an Nurse Practitioner
" Proficiency in computer skills and data management required (e.g. Excel. Word. Access)
" Excellent organization, communication and interpersonal skills
" Required to travel between LTC homes and WOHC campuses
" Excellent critical thinking, problem solving and organizational abilities
" Excellent interpersonal skills with all members of the team including patients and families
" Evidence of well developed conflict management skills
" Demonstrated ability to deal effectively with sensitive issues with families and staff
" Demonstrated knowledge and/or experience working within a professional practice and patient focused care environment
" Evidence of initiative in ongoing knowledge development and reflective practice
" Will be required to work at Long Term Care Homes within Central West
" Fluent in English (comprehension, reading, writing, and speaking)

JOB SUMMARY:
The NP will demonstrate a high level of autonomy and expert skill in formulating clinical decisions and appropriately managing a population of patients within Central West LTC facilities. The principle responsibilities may include: utilizing and demonstrating a comprehensive theoretical knowledge base and advanced level of clinical competencies, collecting and interpreting data about the health of the patient; determining a secondary diagnosis and treating complex responses of individuals and families to actual or potential problems; playing a leadership role within the team; involvement in quality improvement initiatives; participating in policy and procedure development activities/programs; acting as resource and serving as a consultant to individuals and groups within the professional community and other hospital/agencies; providing educational support to patients, families and staff.

This position is accountable to the Coordinator of the Program and the Director of the Seniors Health System.

The NP role involves close collaboration with stakeholders to facilitate change and promote staff and patient/family satisfaction within a philosophy of patient-centered care. In that context, the NP performs various functions including participating in the development of quality management and evaluation tools, as well as enhancing long term care staff skills in the area of client/resident education.
