POSITION: NON-UNION
Clinical Pharmacy Specialist: Medication Utilization - One (1) Full-time

DEPARTMENT/CAMPUS:
Pharmacy Program - William Osler Health System

POSTING NUMBER: 131145

HOURS: Currently Monday to Friday, Days
(subject to change in accordance with operational requirements)

QUALIFICATIONS:
" Current licensure or eligible for licensure with the College of Pharmacists of Ontario
" Minimum five (5) years clinical experience in a health care setting preferred
" Hospital Pharmacy Residency or Advanced Credentialing preferred
" Strong data management skills
" Strong computer skills both in application and utilization
" Strong collaborative and consultative leadership style
" Excellent interpersonal and communication skills
" Demonstrated commitment to practice advancement
" Strong organizational and time-management skills
" Demonstrated ability to work as an effective member of a multidisciplinary team
" Proven leadership skills
" Required to work and travel to all Hospital Campuses

JOB SUMMARY:
This is a Corporate Role reporting to the Corporate Director, Pharmacy Program; you will be responsible to:
" Develop and implement drug utilization management strategies that allow for assessment and improvement of the quality of drug use including the development and implementation of guidelines and protocols for appropriate and cost effective medication utilization (e.g. Policy & Procedures, automatic-substitutions, drug utilization evaluations and statistical analyses, etc);
" Support clinical programs that improve the quality, effectiveness & efficiencies associated with medication management (including provision of pharmaceutical care);
" Identify changes in drug therapies and practice guidelines that are supported by literature and peer evaluations;
" Perform audits to address program specific medication utilization liaising with medical, pharmacy and other health care professionals in development of action plans to address identified inefficiencies;
" Maintain a workload measurement system for drug utilization activities;
" Ensure that William Osler Health System and the Pharmacy Program is in compliance with standards relevant to medication management (e.g. ISMP, CSHP, etc);
" Assist the Corporate Director and the Pharmacy Managers in the budget process;
" Participate in the education of pharmacists, pharmacy undergraduate and graduate students, other healthcare professionals and medical staff as required;
" Support Pharmacy & Therapeutics Committee through Formulary Management by applying principles of pharmacoeconomics and drug use reviews;
" Oversee the on-going maintenance of the Osler Medication Formulary and the Parenteral Manual;
" Provide assistance with medication information requests to clinical staff
" Participate in other committees as appropriate (e.g. the Regional DUE meetings, etc)
" Perform other duties as assigned.
