Job Description

POSITION:
Complex Diabetes Clinic Coordinator - One (1) Full Time

DEPARTMENT/CAMPUS:
Centre for Complex Diabetes Care (CCDC) - Brampton Civic Hospital

QUALIFICATIONS:
" BScN or Bachelors Degree in Food or Nutrition
" Registered with the College of Nurses of Ontario or registered with the College of Dietitians of Ontario
" Certified Diabetes Educator
" Minimum of three (3) years Diabetes Education experience
" Additional educational preparation and experience in Clinical Leadership an asset
" Ability to communicate effectively both verbally and in writing with interdisciplinary team, patients and their families
" Excellent organizational and interpersonal skills
" Ability to multitask and function in a fast paced environment
" Computer skills in Microsoft Office
" Excellent attendance, punctuality and work record

RESPONSIBILITIES:
" Participation in the orientation, teaching and mentoring of new staff to reflect best practices
" Coordination and problem solving in day to day clinic function
" Promoting good interpersonal relationships by modeling professional behaviour and practice
" Identifies staff performance issues and consults with the CCDC Manager
" Participates with staff in assessing the need for supplies and equipment repairs and makes recommendations to CCDC Manager
" Facilitates the implementation of new or revised policies and educational programs
" Chairs team meetings, sending out agenda and meeting minutes
" Coordinates staff education, teleconference sessions, meeting with manager and communicates messages from manager
" Coordinates Heart & Stroke HTN MGT Program and works with CCDC team to develop, implement and carry out Chronic Disease management programs
" Program planning work, i.e. trial Meditech Build, train staff on electronic charting
" Payroll and staff scheduling, vacation and education requests
" Triage CCDC referrals
" Make calls to patients regarding their prep required and to labs and physician offices
" Coordinate with Data Entry Clerk to have data entered
" Ensure all appropriate correspondence is sent out (i.e. letters to family physicians for patients with missed appointments)
" Track patients who have not followed up in CCDC and ensure they are offered an appointment or are discharged and their physician is notified
" Coordinate completion/faxing/collection/analysis of patient and physician satisfaction surveys with secretary and data entry clerk
