Job Summary
The Nurse Practitioner (NP) - Geriatrics, works with the inter-professional team and physician partners to meet the unique needs of patients requiring geriatric consultation. The NP provides leadership and promotes excellence in nursing through the roles of practitioner, consultant, educator, researcher, and leader. By utilizing theoretical and clinical expertise in this specialty area, the NP delivers advanced practice nursing to patients and families with complex situations within the in-patient and community/ambulatory settings. The NP contributes to the education and professional development of nursing/interdisciplinary staff, students and the professional community within these areas of specialization, and participates in academic and research projects. The NP - Geriatrics integrates research into practice to ensure best practice standards are maintained and incorporated into clinical initiatives.

" Current registration in the Extended Class with the College of Nurses of Ontario required
" Masters Degree in Nursing required
" NP Primary Care or NP Acute Care with an interest in Geriatrics preferred
" CNA Specialty Certification in Gerontology preferred
" Eligible for cross-appointment to an academic institution preferred
" Membership in related professional associations required, e.g. (NPAO, RNAO, Geriatric Nursing Association membership, etc) is preferred
" Innovative, creative and professional role model
" Ability to prioritize/organize multiple demands
" Ability to work autonomously
" Proficient with Microsoft Office applications including Outlook, Word, Excel, PowerPoint
" Able to meet the physical demands of the position

Responsibilities:
The Nurse Practitioner (NP) - Geriatrics will utilize a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for both inpatients and outpatients. This will involve health assessment and diagnosis of medical and cognitive problems, developing and implementing a plan of care, providing health counseling and emotional support in the areas of health promotion and the prevention of illness, injury and complications. The NP will work in collaboration with consulting Geriatricians to facilitate early assessment and intervention for patients requiring geriatric consults.
The NP role involves close collaboration with stakeholders to facilitate change and promote staff and patient/family satisfaction within a philosophy of patient-centered care. In that context, the NP performs various functions including participating in the development of quality management and evaluation tools.


Clinical Practice:
" Uses theoretical and evidence-based practice to plan, implement and evaluate advanced nursing care for patients within the Geriatric program
" Obtains and documents a comprehensive health history, performs a physical examination on patients requiring acute geriatric assessment, collaborates closely with the Geriatricians and other members of the Interdisciplinary team, and coordinates care involving other healthcare providers.
" Prescribes and monitors drug therapy and orders and interprets appropriate diagnostic and laboratory tests within the current legislative framework and internally developed and approved authorizing mechanisms such as medical directives
" Leads communication between patients and health care team when clinical, ethical or moral issues arise
" Partners with patients and families to ensure their priorities and needs are being addressed by the health care team
" Addresses gaps and duplication of services within the system to enhance patient access to care and flow of services

Consultation/Collaboration:
" Provides consultation internally and externally for clinical practice issues within the NP's area of specialty and scope of practice. Consults with physicians, and refers to specialists and other team members when appropriate
" Collaborates and participates in planning, promoting, implementing and evaluating new ideas, concepts, and interventions to facilitate care to patients and families

Leadership:
" Participates in professional, program and hospital-wide initiatives
" Actively promotes professional role through participation in external forums such as the Nurse Practitioner Association of Ontario
" Advocates to influence health care practice, policy and outcomes
" Serves as a resource and consultant to administration in decision-making, strategic planning and problem-solving processes
" Examines current processes, identifies opportunities for change and provides recommendations for quality improvement as necessary
" Provides leadership in the development/revision of practice guidelines, protocols, policies and procedures
" Promotes the integration of the nursing viewpoint in program planning and problem solving within the team and the organization

Education:
" Functions as a content expert in their specialized field of practice and acts as an expert resource to the inter professional team
" Actively pursues academic cross appointment and participates with academic institutions as a clinical expert
" Prioritizes health promotion and disease/complication prevention when educating patients and families
" Acts as a clinical practice mentor and role model and facilitates integration of evidenced-based practice and clinical practice guidelines into practice
" Enhances personal and professional development through reflective practice and by attending educational opportunities and /or conferences in clinical specialty, research & scholarship, and/or leadership topics

Research
" Facilitates the utilization of research by nursing and other inter professional staff
" Encourages, supports, and facilitates the development of researchable questions in clinical practice
