Diversity Coordinator: Language/Cultural Support for Independent Therapies and Dialysis Access
**One (1) Temporary Full Time position for approximately 18 months**
DEPARTMENT/CAMPUS: Brampton Civic Hospital
Hours: Mostly days but occasional evenings and weekends may be required
Travel: Will be required to travel in between hospitals and in the community

JOB SUMMARY: With support from the Ontario Renal Network (ORN) and in partnership with Halton Healthcare and Trillium Health Partners, William Osler Health System is seeking an individual to develop an innovative and culturally appropriate model of care to serve the diverse needs of populations for the purposes of increasing access to independent modalities and dialysis. This will help support the ORN and the Local Health Integration Network (LHIN) objective of shifting to a 'home first' philosophy within this patient population.
The successful candidate will be working across the Mississauga Halton and Central West LHINs and reporting to a core committee from the Halton Healthcare, Trillium Health Partners and William Osler Health System.
Minimum Qualifications & Experience:
" College Diploma or University Degree in Health Management/Administration
" Formal training and experience in project management
" Formal training and experience in diversity and equity
" Experience of the health sector, barriers to health care and disparities for vulnerable populations
" Minimum three years experience working with diverse communities/vulnerable populations
Excellent Knowledge of:
" Diversity/equity principles, concepts and sensitivities in relation to health care/high risk populations
" Project management and conducting environmental scans/assessments
" Excellent communication skills
" Good knowledge of the health care needs of diverse communities
" Delivering presentations
" Consultation, client service and relationship management
" Ability to work independently and meet deadlines with minimal supervision
" Computer skills including PowerPoint
" Excellent report writing
" Work with a core committee to develop an 18 month action plan
" Complete an environmental scan outlining the present state of cultural and language barriers to access accessing independent (home) dialysis and access to appropriate care
" Develop a patient satisfaction questionnaire
" Engage appropriate stakeholders - internally across the hospitals, the LHINs and in the community
" Implement the project plan, initiatives and monitor indicators
" Compile a compendium of culturally competent and language support resources and model of care available to renal teams across the Mississauga Halton and Central West Local Health Integration Networks
" Develop education material that are culturally and linguistically appropriate and address the barriers identified through the environmental scan
" Develop an outline of a culturally and linguistically appropriate patient care model for independent modalities and dialysis access creation/care that can be shared provincially
" The model to include partnerships with community agencies and interpreter services
" Develop an outline of a diversity training module and training materials for staff involved in independent modalities and access creation
